Position Summary The BRIDGES/Adult Transition Program Nurse Practitioner (NP) will provide comprehensive, age-appropriate care to adolescents, young adults, including young adult and adult survivors of chronic illness. The NP will work across inpatient, ambulatory, as well as preoperative settings at Boston Children's Hospital. Working collaboratively with NPs, physicians, and interdisciplinary team members, the NP will provide direct clinical care and serve as a clinical consultant on the inpatient service with expertise in the unique healthcare needs of the adolescent and young adult population, with a focus on pulmonary complications. A key component of this role includes preoperative assessment and planning for adult patients receiving care at Boston Children's Hospital, including preoperative chart review and participation in preoperative adult clinic visits. The NP will collaborate with interdisciplinary teams to identify medical and age-related considerations, optimize patients for procedures, and support safe, coordinated care throughout the perioperative experience. The NP will also play an important role in supporting adolescents, young adults, and their families as they navigate the transition from pediatric to adult-oriented healthcare. This includes facilitating continuity of care and assisting patients in identifying and establishing an appropriate adult medical home. Key Responsibilities Provide comprehensive, age-appropriate inpatient clinical care for adolescent and young adult patients, including survivors of chronic illness, while collaborating with interdisciplinary teams to develop and manage individualized plans of care with a focus on pulmonary complications. Serve as a clinical consultant across inpatient and ambulatory settings regarding the care of adolescent and young adult patients, including those with complex and chronic health conditions. Participate in preoperative assessment and planning for adult patients, including comprehensive chart review to identify relevant medical history, comorbidities, medications, previous procedures, and potential perioperative considerations. Conduct and/or participate in preoperative adult clinic visits, collaborating with patients, families, surgical teams, anesthesia, and other specialists to support safe and appropriate procedural planning. Guide patients and families through the transition to adult healthcare while facilitating communication and coordination across pediatric and adult care teams. Participate in patient and family education, care coordination, and transition-planning activities. Provide education and clinical guidance to members of the healthcare team regarding best practices in adolescent and young adult care and healthcare transition. Collaborate with interdisciplinary partners to identify barriers to successful healthcare transition and develop individualized strategies to address patient and family needs. Schedule 24-hours/wk., (2) 12-hour shifts, 7:00 a.m.-7:30 p.m. Flexibility to accommodate (3) 8-hour shifts/wk. based on program and candidate needs. Monday through Friday, no weekends, holidays, or overnight shifts. Minimal Qualifications Education Master of Science in Nursing Experience Minimum of 5 years of nursing experience in an inpatient pediatric setting. Experience caring for young adult patients preferred. Experience supporting the transition of adolescents and young adults to adult healthcare preferred. Strong clinical, communication, collaboration, and care coordination skills. Licensure/Certifications Current MA Nurse Practitioner licensure. Current Family Nurse Practitioner (FNP) or Adult Nurse Practitioner (ANP) certification required. Additional licensure, prescribing authority, and credentialing requirements must be maintained in accordance with Boston Children's Hospital and applicable regulatory requirements. bchAPP The posted pay range is Boston Children's reasonable and good-faith expectation for this pay at the time of posting. Any base pay offer provided depends on skills, experience, education, certifications, and a variety of other job-related factors.
