Summary:

The Supervisor of the Lend a Hand Care Management Program oversees day-to-day program operations and supports a high-performing team of Social Workers and Community Health Workers focused on improving patient outcomes and addressing social determinants of health. This role provides leadership, staff development, and operational oversight to ensure delivery of high-quality, efficient, and patient-centered services.

The supervisor partners closely with internal and external stakeholders, serving as a key resource for problem-solving, workflow optimization, and care coordination. Responsibilities include supporting staff performance management, training, and professional development, as well as assisting with staffing needs such as scheduling and hiring.

The Supervisor promotes a culture of accountability and continuous improvement through data-informed decision-making, helping the team achieve program goals and strengthen care delivery for high-risk populations.

Required Minimum Knowledge, Skills, and Abilities (KSAs)

1. Education: Bachelor's degree in health care related field preferred. Relevant experience in related field will be considered in lieu of a degree.
2. License/Certifications: N/A
3. Experience: See "Education." A minimum of 3 years progressively responsible work experience preferably in a field relevant to the identified program is required. Supervisory experience strongly preferred. Basic knowledge of healthcare and social service systems, with previous experience in healthcare and/or social service field. Customer service skills and experience preferred.
4. Strong interpersonal and communication skills required to interact effectively with all team members, clients from diverse backgrounds, and stakeholders. Communication includes written materials and correspondence.
5. Judgement and ability to resolve problems in an accurate and timely manner.
6. Knowledge of project management, and educational practices.
7. Strong software and computer skills, including Microsoft Office (including Power Point, Access, and Excel), database and website utilization and/or development.
8. Ability to work both independently and to collaborate with teams of individuals in diverse settings.
9. Analytical ability to gather and interpret data in situation where the problems are moderately complex.
10. Ability to concentrate and pay close attention to detail.
