Description Management/Leadership Summary: The Supervisor of the Lend a Hand Care Management Program oversees day-to-day program operations and supports a high-performing team of Social Workers and Community Health Workers focused on improving patient outcomes and addressing social determinants of health. This role provides leadership, staff development, and operational oversight to ensure delivery of high-quality, efficient, and patient-centered services. The supervisor partners closely with internal and external stakeholders, serving as a key resource for problem-solving, workflow optimization, and care coordination. Responsibilities include supporting staff performance management, training, and professional development, as well as assisting with staffing needs such as scheduling and hiring. The Supervisor promotes a culture of accountability and continuous improvement through data-informed decision-making, helping the team achieve program goals and strengthen care delivery for high-risk populations.
