Overview

The Patient Care Coordinator (PCC) is a dedicated clinical support role that partners directly with physicians to improve clinic efficiency, reduce administrative burden, and enhance care coordination.

The PCC supports providers during patient visits and manages key non-clinical responsibilities between visits, enabling physicians to focus on direct patient care.


Responsibilities

In-Clinic (During Patient Visits)

  • Prepare patient charts prior to visits, including review of labs, imaging, and relevant clinical documentation
  • Accompany providers during patient visits, as appropriate
  • Perform real-time medication reconciliation and update medication lists
  • Enter orders (labs, imaging, medications, referrals) for provider review and signature
  • Retrieve clinical information (labs, imaging, notes) during visits as needed
  • Support clinic efficiency by managing visit-related tasks in real time

Between Visits & Care Coordination

  • Monitor and manage EMR messages, patient portal communications, and provider correspondence
  • Serve as a primary point of contact for patients for non-clinical questions and coordination needs
  • Coordinate scheduling adjustments in collaboration with scheduling teams (e.g., provider availability, urgent needs)
  • Track follow-up items, including labs, imaging, referrals, and external records
  • Support care coordination across departments and services
  • Provide patient education on communication expectations and how to contact the care team

Collaboration & Team Support

  • Partner closely with nursing staff to ensure smooth handoffs and appropriate escalation of clinical concerns
  • Support triage workflows by managing administrative and coordination tasks
  • Participate in daily huddles, workflow optimization, and process improvement initiatives
  • Provide cross-coverage for other PCCs as needed
  • Other duties as assigned

Qualifications

  • Demonstrated experience in outpatient clinical workflows
  • Strong EMR proficiency, including comfort with order entry and documentation
  • Excellent organizational, communication, and multitasking skills
  • Proven reliability, professionalism, and discretion

Preferred Qualifications

  • Experience supporting providers directly in a clinical setting
  • Demonstrated interest in career advancement and expanded responsibilities
