Patient Navigator / Care Coordinator

As a Patient Navigator, you'll serve as a trusted guide for patients and families, helping them navigate the healthcare system, access critical resources, and overcome barriers to care. In this highly collaborative role, you'll act as a key liaison between patients, providers, and community partners to ensure seamless, patient-centered care and improved outcomes.
What You'll Do
*Advocate for patients and families, ensuring their needs, goals, and preferences are reflected in care plans.
*Serve as a primary point of contact between patients, providers, and other healthcare stakeholders.
*Coordinate appointments, services, referrals, and follow-up care across multidisciplinary teams.
*Help patients understand their care plans and support adherence to recommended treatments.
*Connect patients with valuable healthcare and community resources, including financial assistance, transportation, and support services.
*Educate patients and caregivers on medical conditions, treatment options, and self-management strategies.
*Empower patients to make informed decisions and actively participate in their healthcare journey.
*Maintain accurate documentation of patient interactions, care plans, and resource referrals.

*Experience in healthcare, care coordination, patient services, social work, case management, or a related field preferred.

This role does not provide direct patient care but is instrumental in supporting patients and families through every step of their healthcare experience.

Qualifications:

  • Bachelor's Degree in related field of study required; relevant experience can be considered in lieu of degree.

  • 1+ years of patient facing, medical office or other related experience strongly preferred.

  • Bilingual Spanish speaking strongly preferred.

Additional Skills for Success:

  • Strong knowledge of healthcare resources, community services, and patient advocacy.

  • Excellent communication and interpersonal skills.

  • Ability to collaborate effectively with healthcare professionals across multiple disciplines and experiences.

  • Exceptional organizational and time management skills.

  • Familiarity with electronic health records and case management software.

We're seeking a compassionate, resourceful professional who excels at building relationships, navigating complex healthcare environments, and helping patients access the care and support they need.

Schedule and Work Model

  • Full-time, Monday through Friday, approximately 8am-4:30pm.

  • Hybrid work model: On-site at Massachusetts General Medical Group (Staniford Street in Boston, MA) 4 days per week; remote/work from home 1 day per week.

  • Remote workdays require stable, secure, quiet, compliant workspace with MGB issued equipment.
