Type of Opportunity: Full time Job Exempt: No Job is based: Presbyterian Santa Fe Medical Center Work Shift: Weekday Schedule Monday-Friday (United States of America)Responsibilities: As a Community Health Worker supporting the Santa Fe NM area you will make a meaningful impact on the health of children and their families. This role is embedded in Presbyterian Santa Fe Medical Center, connecting patients to community resources, providing basic health education, supporting care navigation, and co-leading the Produce Rx program for perinatal and pediatric participants. Qualified candidates bring experience in community health or clinical settings, a commitment to community wellbeing, and comfort using Microsoft Office and learning an electronic medical record. This position works primarily in person at the clinic, with some outreach in Santa Fe and surrounding communities. It is part of a collaborative statewide Presbyterian Community Health team, with remote meetings and occasional travel for trainings. This role includes a $4/hour differential to support costofliving needs in the Santa Fe area. Key responsibilities include:
