ElderServe Health is a mission-driven, nonprofit managed care organization serving the New York metropolitan area. ElderServe provides long-term services and supports to more than 20,000 older adults and adults with disabilities across New York City, as well as Westchester, Nassau, and Suffolk Counties. ElderServe primarily serves a frail and elderly membership who is predominantly dual-eligible and enrolled in Medicaid managed long-term care, Medicare-only and integrated Medicaid-Medicare plans. The CareSource and ElderServe affiliation addresses these critical long-term care needs to help members be as healthy and independent as possible.

Job Summary:

The Remote On-Call Nurse (RN) ensures member safety and clinical continuity during after-hours by providing real-time clinical assessment, triage, and care coordination remotely. Utilizing strong clinical judgment, the RN evaluates symptoms, determines acuity, and guides callers on appropriate next steps, including home care instructions or emergency intervention. As the primary clinical contact during evenings, weekends, and holidays, the RN documents interactions in the electronic health record and facilitates smooth transitions between on-call and daytime operations, all while promoting member well-being and delivering high-quality, patient-centered care.

Essential Functions:
  • Provide remote triage via phone or virtual platforms, using evidence based clinical protocols.
  • Assess members' symptoms and determine the appropriate level of care, including home care instructions, urgent escalation, or emergency response.
  • Document all encounters accurately and promptly in the electronic health record.
  • Communicate with on call providers, interdisciplinary teams, or emergency services as needed.
  • Offer support and education to members and families to promote safety and avoid preventable ED visits.
  • Manage after-hour issues related to member needs, care coordination, and service interruptions.
  • Maintain awareness of organizational policies and escalation of workflows for urgent situations.
  • Participate in handoff communication to daytime clinical teams to ensure continuity of care.
  • Perform additional duties assigned to support 24/7 operations.
  • Perform call coverage as needed.
  • Perform any other job related duties as requested.

Education and Experience:
  • Associates degree required
  • Bachelor's degree preferred
  • Two (2) years experience working in an acute care setting required
  • Two (2) years experience working in home care and MLTC/MAP required
Competencies, Knowledge and Skills:
  • Ability to define problems, collect data, establish facts, and draw conclusions
  • Must be able to read, write, and speak English
  • Ability to communicate effectively with a diverse group of individuals
  • Knowledge of local, state and federal healthcare laws and regulations and company policies regarding care management practices
  • Adhere to code of ethics that aligns with professional practice
  • Critical listening and thinking skills
  • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals
  • Convey a positive attitude by being friendly, kind and interested in helping others
  • Knowledge of Medicare and Medicaid regulations
Licensure and Certification:
  • Current, unrestricted Registered Nurse licensure in the state of New York required
  • Case Management Certification preferred
Working Conditions:
  • Fully remote position - all work performed from a homebased setting
  • Requires sitting for extended periods while taking calls or documenting
  • On call work includes evening, weekend, and holidays
  • Must be able to respond to escalations promptly during assigned on call shifts
  • Requires use of standard office equipment (computer, headset, phone systems, etc.)
  • Travel is not typically required

Compensation Range:
$62,700.00 - $100,400.00 CareSource takes into consideration a combination of a candidate's education, training, and experience as well as the position's scope and complexity, the discretion and latitude required for the role, and other external and internal data when establishing a salary level. In addition to base compensation, you may qualify for a bonus tied to company and individual performance.
