Purpose of Job This position is responsible for the delivery of individualized patient care through the nursing process of assessment, planning, implementation and evaluation in accordance with the Standards of Nursing Care and Practice as defined by Ridgeview Medical Center, the Minnesota Nurse Practice Standards Act and the ANA Standards of Clinical Nursing Practice. Job Functions
Admits, recertifies and discharges all patients on their census unless on weekends, and performs most, if not all follow up visits.
Responsible for obtaining authorization from appropriate payor for all visits made.
Assesses patient health problems and needs and develops and implements patient care plans accordingly.
Updates care plan and medication changes as they occur and completes all documentation according to agency guidelines.
Attends care conferences (remote or face to face) as directed.
Advises and educates patients, family members and significant others on health maintenance and disease prevention for the patient.
Participates in quality improvement activities as appropriate and monitors outcomes.
Serves as a patient advocate in seeking appropriate community resources to meet patient needs.
Interacts with and contributes to the professional development of peers, other healthcare providers and colleagues to enhance own professional nursing practice.
Demonstrates competencies in the specialty area of Hospice.
Performs other duties as assigned.
Minimum Education/Work Experience
Knowledge/skills/Abilities
Demonstrate critical thinking and decision making
Ability to meet and maintain the necessary background checks as aligned with position functions
Ability to communicate in the English language for effective written and verbal correspondence in order to complete job functions as mentioned above
Capable of making independent decisions with the use of good critical thinking and problem solving skills
Ability to work and multi-task in a high stress and fast paced environment
Ability to work autonomously and as a team
Ability to respond appropriately to unpredictable situations and diverse patient populations
Ability to communicate in the English language for effective written and verbal correspondence in order to complete job functions as mentioned above
License/Certifications
Current professional nurse licensure in Minnesota or if licensed in another state, a permit from the Minnesota Board of Nursing while the license application is in process
Valid MN driver's license and meets Ridgeview's insurability requirements
Current Healthcare Provider CPR certification
