Under the direction and supervision of the Director of Health Equity, the Cancer Screening Navigator/Project Manager will focus on guiding individuals through the cancer screening process, ensuring timely and efficient access to care, and supporting them throughout their journey,

This role involves patient education, care coordination, and the identification and processing of social, financial, and logistical barriers that may prevent patients from accessing or completing screenings and follow care,

Navigator will facilitate appointments, track results, and ensure timely follow-up for abnormal findings,

Develop and implement strategies to increase cancer screening rates and ensure adherence to recommended follow-up procedures,

This role also builds relationships with healthcare providers and community organizations to promote participation in screening programs,

In addition, lead navigator will maintain accurate records of patient interactions, data, and outcomes to track program effectiveness and identify areas for improvement,

This role will coordinate and manage projects, contribute to the development and implementation of cancer screening programs and initiatives, and support other areas of navigation as needed,
