Provides support to the Patient Education Coordinator in educating non-hospitalized individuals with diabetes. This includes direct patient education, collaboration with healthcare providers, and development of educational resources and departmental goals.

I. Major Responsibilities:

1. Maintain current knowledge of patient education and diabetes-related topics through continuing education and professional development.
2. Assist the Patient Education Coordinator in developing departmental goals, systems, and tools for patient and diabetes education.
3. Support hospital and VNA staff in educating their patient populations by assessing learning needs and developing resources.
4. Assess patient knowledge, learning styles, and needs, and provide education using adult learning principles.
5. Maintain accurate records of patient education, including assessments, goals, interventions, and outcomes.
6. Communicate education activities and patient progress to the Patient Education Coordinator.
7. Collaborate with physicians and dietitians in managing referred patients, including identifying trends and communicating outcomes.
8. Foster a respectful and communicative environment that promotes learning.
9. Maintain cost-consciousness in educational activities, including inventory management and budget input.

II. Position Qualifications:
License/Certification/Education:

Required:
1. Graduate of an accredited school of nursing (RN preparation)
2. MA RN license
3. Certified Diabetes Educator (CDE)
4. Current CPR certification
Preferred:
1. BSN

Experience/Skills:
Required:
1. Experience teaching non-hospitalized individuals with diabetes
