Care Coordinator - Alaska Residents only

Care Management | Exempt

We're seeking a Care Coordinator to support timely discharges and smooth care transitions through collaboration with providers, community agencies, and internal care management teams. This role plays a key part in improving patient outcomes through coordination, education, and evidence-based care planning.

This position is a .75 FTE position which means 30-32 hours per week. Full benefits apply (paid time off prorated).

This position is located in Alaska. The employee can be located anywhere in Alaska as this is a remote role within Alaska.

Key Responsibilities

• Facilitate care transitions and discharge planning

• Coordinate services with providers and community resources

• Conduct limited telephonic family assessments

• Support providers and internal clinical teams

• Apply evidence-based case management standards

Qualifications

• MA/MS in a related field or equivalent experience

• Active RN license or licensure in a relevant mental health/human services field

• 2+ years of clinical experience (care coordination experience preferred)

Compensation
$80,000 - $100,000 annually, based on experience, qualifications, and geographic location. Will be prorated to meet .75 FTE.
