Virtual Hospitalist, Advanced Care at Home - Parkline - Chapel Hill, NC

Command Center - Parkline - is considered "place of business" for this role located in Chapel Hill, NC

The Virtual Hospitalist of UNC Health's Advanced Care at Home program is a dynamic, experienced provider who is passionate about transforming the future of healthcare. Under the program, UNC Health physicians and nurses will monitor and communicate with patients in their homes 24/7 from a clinical command center located in Chapel Hill, NC. The virtual team will deploy a network of rapid response home based services to patients as needed. Patients will receive in person assessments by nursing, paramedics and NPs, as well as virtual assessments by nurses and physicians. This role provides the following opportunities:

*Work in a team environment to provide high-quality inpatient care to patients in their own homes through Advanced Care at Home (ACH).

*Clinical outcomes are equal to or better than inpatient hospitalization with improved patient experience

*State-of-the-art equipment at Command Center and patients' home

*Patients have access to hospital services and more including medication management, mobile imaging, home infusion, meal delivery, nutrition consults, phlebotomy, & rehab services

*Integrated care with primary care providers, specialists & ancillary departments

Responsibilities

*Lead a team of Advanced Practice Providers and nurses to coordinate care for patients in ACH

*Perform the initial virtual history and physical for new patient admissions after patient arrives home

*Provide daily virtual visits from the clinical command center in Chapel Hill

*Collaborate with the Advanced Practice Providers to develop and implement a plan of care, including diagnostic and specialty consultations virtually or as day trips to brick and mortar facilities

*Provide safe handoffs to the colleagues and Command Center Registered Nurse, as well as patient's primary care physician during hospitalization as needed and at discharge

*Perform clinical screenings on eligible patients at eligible brick and mortar facilities

*Communicate with the patient and family concerning the ACH model, incorporating patient and family values and goals in the plan of care

*Ensure census growth by identifying appropriate patients for the ACH model which includes patient chart reviews, outreach to brick-and-mortar provider teams, and coordination with internal acquisition team

*Keep abreast and be willing to maintain flexibility as program changes and evolves *Practice in accord with practitioner guidelines per state and hospital regulations

*Command Center - Parkline - is considered "place of business" for this role located in Chapel Hill, NC.

Qualifications

*MD/DO

*Minimum 3 years' experience as an attending physician

*Board Certified/Board Eligible in internal medicine or family medicine

*Committed to providing high quality, evidenced based care to a diverse patient population

*Prior experience with Epic EMR recommended
