Acentra Health is looking for a PASRR Evaluator to join our growing team.

Job Summary

Make a meaningful impact in your community-join Acentra Health as a Clinical Evaluator (PASRR) PRN role in Los Angeles. As a Clinical Evaluator, you will conduct face-to-face PASRR Level II assessments to determine appropriate placements and next levels of care for individuals with complex needs. You will be responsible for completing clinical evaluations for individuals with Serious Mental Illness (SMI), Intellectual Disabilities (ID), Related Conditions (RC), or geriatric populations across nursing homes, hospitals, and community-based settings. These evaluations are crucial in determining the next level of care, improving patient well-being, and supporting better health outcomes.

This PRN Clinical Evaluator position offers both flexibility and purpose-combining remote work from your home office with local travel. Acentra Health covers and reimburses travel-related expenses, including mileage and parking. Additionally, we provide all necessary technology and equipment for your home office setup, including a laptop for use while in the field.

Responsibilities

  • Conduct PASRR Level II assessments by reviewing relevant records, gathering collateral information, and evaluating the individual's physical, behavioral, and psychological functioning to support informed clinical decisions.
  • Responsible for conducting Preadmission Screening and Resident Review assessments and developing informed level-of-care recommendations, and determining the most appropriate next level of care for those who may require long-term care placement.
  • Review medical records, and interact with healthcare professionals, patients, their families, and other support systems to conduct accurate screenings, evaluations and determinations.
  • Provide well-informed, clinically sound Level of Care recommendations based on comprehensive evaluations.
  • Demonstrate strong knowledge of standards of practice for Level of Care determinations, as well as mental health, geriatrics, and other conditions commonly requiring long-term care placement.
  • Ensure compliance with required workflows, including re-reviewing records based on input from the psychiatrist.
  • Determine appropriate clinical diagnoses that account for psychological and behavioral factors.
  • Use computer equipment and web-based software to complete documentation and conduct work.
  • Understand, and comply with corporate policies, including those related to HIPAA and its Privacy and Security Rules.

Qualifications

Required Qualifications and Experience

  • Must possess an active, unrestricted license in the State of California in a healthcare or related field, such as Licensed Clinical Social Worker (LCSW), Licensed Master Social Worker (LMSW), Licensed Marriage and Family Therapist (LMFT), Licensed Independent Clinical Social Worker (LICSW), Licensed Clinical Professional Counselor (LCPC), Licensed Clinical Psychologist (LCP), Licensed Mental Health Professional (LMHP) and Psychiatric Mental Health Nurse Practitioner (PMHNP).
  • Graduation from an accredited college with a relevant degree program and a minimum of 2 - 3 years of experience in assessing individuals with mental health or psychiatric disorders, including complex differential diagnoses related to substance use, medical conditions, cognitive impairments, and dementia.
  • Minimum 1 year of experience working with individuals with intellectual or developmental disabilities or related conditions.
  • Demonstrated expertise in assessing and analyzing an individual's psychological, emotional, behavioral, and physical functioning.
  • Effective communication skills with the ability to interact effectively with individuals and groups through verbal, written, and electronic formats.
  • Experience in medical record abstraction and clinical data review.

Preferred Qualifications

  • Basic knowledge of Preadmission Screening and Resident Review (PASRR) requirements, including compliance with state and federal regulations.
  • Knowledge of Federal Regulation 42 CFR, Chapter 483, pertaining to long-term care and resident assessment requirements.
  • Familiarity with mental health, intellectual and developmental disabilities (IDD), geriatrics, and other conditions commonly requiring long-term care services and supports.
  • Solid understanding of standards of practice related to Level of Care determinations, including those applicable to mental health, geriatrics, and conditions often requiring long-term care placement.
  • Understanding of psychiatric disorders as defined in the current edition of the DSM, in alignment with applicable state guidelines.
  • Proficient in computer-based software and tools, including Microsoft Office Suite (Word, Excel, Outlook, Teams).
  • Experience working with the geriatric population.

Compensation

The compensation for this role is $41.00 to $42.00 hourly

Based on our compensation program, an applicant's position placement in the pay range will depend on various considerations, such as years of applicable experience and skill level.
