The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience.

Job Overview:

The Patient Navigator assists the pulmonary rehab therapists coordinating and facilitating the daily operations of the pulmonary rehabilitation center, in accordance with the philosophy, policies and program plan of the Monmouth Medical Center Southern Campus Pulmonary Rehabilitation Department.

They assist in coordinating efforts of all members of the rehabilitation program including physicians, staff and patients. The Navigator will participate in the initial contacts, follow up and tracking of the care and their progression in the program. The Navigator will collect and analyze data for continuous performance improvement identifying and making recommendations for improved patient outcomes.

In addition, the Navigator is responsible for patient and physician satisfaction and the coordinating of outreach efforts for the medical and lay community including but not limited to educational information sessions, support groups, and marketing efforts.

Qualifications:

Required:

  • High School Diploma or Equivalent
  • Previous patient care or experience in health-related environment

Preferred:

  • Patient Navigator experience

Certifications and Licenses Required:

  • AHA BLS

Scheduling Requirements:

  • 6-hour shifts with variable start times on Monday, Tuesday, Thursday, Friday
  • Part Time Day shifts, 24 hours per week

Essential Functions:

  • Key responsibilities first and move down by importance
  • Assisting with the creation and/or distribution of communications/announcements to hospital staff and/or the public, to promote awareness of the Post-COVID Recovery Program to medical professionals as well as the public at large.
  • Communications as-needed with physician's offices to facilitate receipt of prescriptions for patients to participate with the Program.
  • Coordination of referrals to various services of the RWJBH system and/or affiliated physicians, to which Program patients may be referred for additional health care services.
  • Assistance with transportation arrangements via the hospital or community services, for those patients who are challenged by lack of transportation to the hospital for outpatient services
  • Communication and coordination with hospital Case Management and with hospital ER staff, to obtain information on discharged inpatients as well as discharged ER patients who may benefit from the Post-COVID Recovery Program, and to contact and coordinate enrollment in the Program for those patients
  • Assist with patient consultations and ensures that all education and exchange of data takes place
  • Communicates effectively and seeks direction in timely and appropriate manner.
  • Completes and submits billing and financial information
  • Demonstrates flexibility in response to unexpected changes in work volume, emergency staffing and schedule changes. Positively accepts difficult and unexpected assignments (especially those outside the job description) making sure to clarify expectations and workload priorities
  • Determines priorities and organizes work and time to meet them
  • Documents patient outcomes, tests and any other pertinent issues. Maintains records as required
  • Follows appropriate infection control guidelines by providing routine cleaning of equipment after each use
  • Maintains appropriate level of inventory of patient supplies
  • Performs other duties of the department as assigned
  • Proficiently uses computer system
  • Provide information to patient (i.e. Patient education materials)
  • Provides information to internal/external customers, staff, patients, families and visitors in person and on the phone concisely, accurately and completely, maintains a positive, courteous tone and respecting confidentiality
  • Resolves problems involving staff, patients, families, physicians and other hospital departments
  • Respond initially to all calls and queries made to the office, and direct visitors and calls to appropriate personnel and offices in a timely manner
  • Evaluate patient prior to testing, gathering pertinent clinical history, recent use of bronchodilator, and lab work to apply to each testing session to facilitate appropriate testing, proper diagnosis and reduce the possibility of retesting due to inadequate assessment
  • Coordinates the scheduling of testing with scheduled testing
  • Process reports of pulmonary function testing for physician interpretation the same day that the test is done. Assure that all charges are entered in SCM and completed the same day that tests are done. Place a preliminary copy of inpatient tests on patient chart at the completion of testing
