Licensed Clinical Social Worker (LCSW) - Outpatient Dialysis Clinic

Location: Brockton, MA 02301

Schedule: Full-time, day shifts

Innovative Renal Care is hiring a Licensed Clinical Social Worker (LCSW) to support patients living with chronic kidney disease at our 25-station outpatient dialysis clinic in Brockton. You'll work with a collaborative care team serving both in-center hemodialysis (ICHD) and peritoneal dialysis (PD) patients - helping them manage the emotional and social challenges of dialysis treatment.

Key Responsibilities

• Complete comprehensive psychosocial assessments at admission and annually
• Provide adjustment counseling, education, and emotional support to patients and families
• Coordinate access to transportation, insurance, financial aid, and community resources
• Actively participate in interdisciplinary care planning and care conferences
• Maintain clinical documentation and progress notes in compliance with ESRD guidelines

Qualifications

• Master's degree in Social Work (MSW) from an accredited program
• Active LCSW in Massachusetts (or eligibility for licensure)
• At least 2 years of clinical social work experience (dialysis or ESRD experience preferred)
• Strong communication and care coordination skills
• Committed to patient dignity, ethics, and collaborative care

Salary Range: $31.59 - $41.95 / hour depending on experience and qualifications
