Scope of Position Provision of social work services to inpatients, outpatients, and their families in The Ohio State University James Cancer Hospital. Position Summary Social work services include psychosocial assessments, mental health counseling, crisis intervention, financial counseling, discharge planning, health education and the provision of material resources and linkage with community agencies. Social work services are provided to patients/families to meet their medically related social and emotional needs as they impact on their medical condition, treatment, recovery, and safe transition from one care environment to another. Core competencies include: Psychosocial assessment, counseling and crisis intervention skills, Knowledge of the PASAAR process for extended care facility placement and the assessment and counseling processes involved, knowledge of home healthcare set-up process and skills assessing neglect/abuse, including substance abuse and domestic violence. The AYA SW will analyze trends in psychosocial support, review literature and evidence-based resources, benchmark with other cancer institutions and integrate recommendations from national sources in the planning and implementation of programs and services. This position will also regularly coordinate care with the AYA Program Coordinator at Nationwide Children's Hospital (NCH) to assure all AYA patients have access to these dedicated services. Minimum Qualifications For Hire: Master of Social Work from an accredited Graduate School of Social Work; State of Ohio Social Work licensure (LSW or LISW) or equivalent education/experience. The primary focus of the position is to implement programs that assist Adolescent amp; Young Adult (AYA) patients and families through their healthcare experience at The James and in their survivorship. The AYA social worker (SW) oversees the implementation and execution of outreach efforts, day to day clinical encounters, and evaluation of all AYA services for The James. This person acts as a liaison between physicians and healthcare teams and families for clinical operations and communication support within The James AYA patient population. This position will proactively identify, develop, and maintain key internal and external relationships to ensure that proper processes and timely execution of AYA services for families, children, caregivers, community, and healthcare professionals. |
