Description

Homebase Medical is expanding rapidly into the Tacoma, Washington area. We are currently searching for an experienced Palliative Care Nurse Practitioner. You will be responsible for palliative care in the home setting.

Key Duties:



  • Conduct Palliative Care Visits
  • Comprehensive Assessment skills including pain management, symptom management, patient and family education.
  • Focus on goals of care.
  • Work collaboratively with our interprofessional palliative care team
  • Support after hours, holiday and weekend on-call
  • Support provider relationships with our clients
  • Actively support the achievement of Homebase Medical's Vision and Goals.


Qualifications:



  • Active Nurse Practitioner license in the State of Washington
  • Board certification (Nurse Practitioner)
  • DEA licensure
  • 2 years or more of palliative care experience required
  • CAPC preferred but not required

    • Per-patient rate: $90 for every patient seen
