We are seeking a Certified Addiction Counselor to join our Assertive Community Treatment (ACT) team based in Shelocta, PA, supporting individuals across Armstrong and Indiana Counties. Earn $24.38 / hour. Position Overview As a key member of our multidisciplinary ACT team, you'll play a vital role in supporting adults with serious mental illness (SMI) and co-occurring substance use disorders. This is a community-based role, providing integrated care where it matters most-at home, in the community, and wherever support is needed.
Our Assertive Community Treatment (ACT) model is built on a team-based approach to serving individuals with complex behavioral health needs. Our teams include psychiatrists, nurses, mental health and substance use professionals, employment and housing specialists, and peer support staff. Together, we deliver comprehensive care, therapy, medication management, case coordination, and through frequent home and community visits. This position requires the use of a personal vehicle for client transportation and community-based services.
