Job Description: The Inflammatory Bowel Disease Advanced Practice Nurse will assist the IBD gastroenterology specialists in evaluating and managing outpatients with IBD or suspected IBD. Essential Functions: They will also provide longitudinal care, with direct oversight for stable IBD patients as well as fulfill patients needs in-between appointments that require a practitioner to interpret study results, adjust or initiate immunosuppression medications and other therapies, and coordinate care with referring and collaborating physicians and surgeons on the care team. The role involves comprehensive patient assessment and diagnosis, including performing detailed health evaluations, gathering medical history, conducting physical exams, and ordering and interpreting diagnostic and health monitoring tests related to IBD as well as any coexisting conditions. Treatment management responsibilities include developing and implementing individualized care plans that incorporate medication and nutrition counseling, healthcare maintenance, and coordination with other healthcare professionals. This also entails adjusting or refilling medications particularly immunosuppressive therapies between visits based on patient response, lab monitoring, and side effects, as well as performing prior authorization coverage appeals for necessary medications and procedures to optimize IBD care. The position requires longitudinal follow-up care through regular visits to monitor patient progress and adjust care plans accordingly. Most patients will be seen at RWJUH and RWJBH outpatient facilities, with a smaller number evaluated at infusion centers. Collaboration is key, with close teamwork alongside IBD physicians, healthcare providers, and clinical and office staff to ensure comprehensive care.
