Responsibilities

As a Social Worker and Therapist at PrairieCare, you'll be an essential member of our care team, making a significant impact on the lives of our patients and their families. In this dynamic role, you will assess, plan, implement, and evaluate psychosocial services for a diverse range of patients, including children, adolescents, and adults, particularly those with co-occurring disorders. Your expertise will help enhance the overall functioning and well-being of those you serve.

Responsibilities and Accountabilities:

  • Social Work Expertise: Bring your knowledge and understanding of social work practices and principles to the forefront, ensuring high-quality care for all patients.

  • Comprehensive Assessments: Conduct effective psychosocial assessments and suicide risk evaluations, contributing to discharge planning and leading psychoeducational and therapeutic groups.

  • Patient and Family Engagement: Communicate openly with patients and their families, keeping them actively involved in treatment plan decisions and fostering a collaborative environment.

  • Care Coordination: Coordinate care with outpatient providers during patient admissions and discharges, ensuring a seamless transition and continuity of care.

  • Clinical Insight: Provide a unique clinical perspective to the interdisciplinary team, drawing on your training in family systems and social work theory to inform treatment strategies.

  • Therapeutic Facilitation: Lead group, individual, and family therapy sessions, creating a safe and supportive space for healing and growth.

  • Coverage Support: Be available to step in and provide coverage for fellow social workers and therapists as needed, ensuring consistent support for our patients.

  • Effective Communication: Maintain clear and effective communication with clinical team members and directors, fostering collaboration and enhancing patient care.

  • Supervision Responsibilities: If assigned, provide supervision for those pursuing clinical licensure, sharing your knowledge and expertise to support their professional development.

  • Cultural Competence: Stay oriented and educated in gender, cultural differences, and age-appropriate care to meet the diverse needs of our patient population.


Qualifications

  • Master's degree and licensed as a social worker in MN (LGSW)
  • Two years previous mental health experience strongly preferred.
  • Experience using electronic health records preferred
  • Cultural responsiveness and bilingual language skills strongly preferred
  • Ability to maintain required malpractice insurance
  • Proficient in Microsoft Office products including Outlook and Word.
  • Able to effectively and efficiently communicate in written and verbal form
  • Ability to smoothly and rapidly shift between dissimilar tasks

    Ability to respond appropriately in times of mental health crisis.

Compensation for this role ranges from $29- $34 per hour depending on education and experience.

An individual's pay rate within the posted range is determined by several factors, including skills, experience, education, and market conditions. Additionally, we consider internal equity to ensure fairness within our organization.
