Position Title: Resource Specialist Work Location: Broward County, Florida (Remote + Field Visits) Assignment Duration: 3 Months (Contract to Hire) Work Arrangement: Remote with field travel - home visits, hospital/group home visits, and community meetings Position Summary: Provides resource support to members and connects members to local resources for various services (e.g., employment, housing, independent living, justice, foster care) based on care/service assessment and plans Works with leadership to identify the best, most appropriate local resources for member's needs based on industry best practices Key Responsibilities: Helps with assessing member needs and develops or contributes to the development of care plans/service plans Assists with connecting members to local resources, obtaining documentation, and supporting applications Acts as a resource for members seeking local services support Monitors care/service delivery and performs follow-ups with members, caregivers, and providers Works with providers, community-based organizations, stakeholders, and government entities Assists in developing training programs for staff and providers Supports in drafting materials and developing local resource service policies and programs Ensures resource services information is documented and updated May conduct telephonic and in-person outreach to locate hard-to-reach individuals Performs other duties as assigned Complies with all policies and standards Qualification & Experience: Requires a Bachelor's degree and 2 - 4 years of related experience Or equivalent experience with relevant skills and knowledge Position is offered by a no fee agency.
