1.0 FTE/40 Hours per Week

The Clinical Pharmacist is responsible for delivering professional and quality pharmacy services across GHC-SCW's pharmacies and clinics in accordance with applicable laws and regulations pertaining to the practice of pharmacy. This role includes direct patient care, interdisciplinary collaboration, health education, participation in quality improvement and clinical pharmacy programs, in addition to retail pharmacy work. Pharmacists at GHC-SCW play a critical role in enhancing medication adherence, safety, and therapeutic outcomes.

The Clinical Pharmacist works under the direction of the Managing Pharmacist, Assistant Managing Pharmacist, Pharmacy Operations Coordinator, or the Clinical Pharmacy Program Manager, and is expected to uphold GHC-SCW's mission, vision, and values in all aspects of practice.

Key Responsibilities



  • Dispense prescription medications and counsel patients on proper use, side effects, and safety.
  • Monitor drug interactions, allergies, and contraindications; ensure safe and effective medication therapy.
  • Provide Medication Therapy Management (MTM) and support chronic disease management initiatives.
  • Collaborate with care teams to optimize medication use and resolve therapy-related problems.
  • Maintain accurate patient profiles and documentation in the electronic medical record (Epic).
  • Educate patients and staff to promote medication adherence and health literacy.
  • Participate in formulary management, clinical projects, and quality improvement committees.
  • Serve as a mentor to pharmacy interns, technicians, and students as applicable.
  • Float to other GHC-SCW locations to meet pharmacy and patient care needs.



Job Specific Minimum Qualifications



  1. Valid license to practice pharmacy in the state of Wisconsin required.
  2. Minimum of one (1) year of experience as a pharmacist preferred.
  3. Excellent customer service skills required. Strong interpersonal skills. Ability to be culturally sensitive and work with a diverse patient population.
  4. Ability to effectively communicate and consult with patients, practitioners and internal staff required.
  5. Excellent verbal communication and listening skills both in-person and on the phone required.
  6. Knowledge of interviewing practices and techniques used to gather information.
  7. Ability to maintain quality, safety and/or infection control standards.
  8. Knowledge of or ability to learn and use computer-based applications such as Microsoft Office, Microsoft Excel, prescription processing software such as Epic Willow and electronic medical record systems such as EpicCare Ambulatory.
  9. Ability to remain current on new trends, pharmaceuticals, pharmaceutical related treatments, disease state management, clinical outcomes and other technologies.
  10. Basic understanding or ability to learn about pharmacy reimbursement/insurance practices.
  11. Ability to work as a member of a team and to maintain good working relationships with patients and staff required.
  12. Ability to pay close attention to detail and problem solve. Good organizational skills.
  13. Ability to maintain patient confidentiality required. Knowledge of HIPAA requirements.
  14. Ability to remain calm and be resourceful in emergency situations required.
  15. Ability to always maintain a professional demeanor.
  16. Ability to float to other pharmacy or clinic locations, as needed. Ability to provide own transportation to various locations, as required.
  17. Ability to bend, reach, lift, pull, push and/or carry up to 20 lbs. regularly required.
  18. Ability to stand for prolonged periods of time required.
  19. Ability to use a keyboard extensively required.
  20. Fine manual dexterity required.
  21. Ability to see near, mid and far range required.
