Scope of Work

Assesses the bio-psychosocial needs and abilities of residents and families, providing a range of Social Work services, interventions and education to meet their needs and enhance abilities. Utilizes personal, family and community resources to achieve and maintain an optimal level of physical, mental, social well-being and quality of life for the residents and families. Ensures the following is completed: bio-psychosocial assessments, care planning, counseling, advocacy for resident and family members; safe and appropriate discharge planning.

Qualifications

Required

• Master's Degree in social casework from an accredited graduate school of social work
• LIC-Master Social Worker (MSW-Master) - STATE_MI State of Michigan
