The LPN is responsible for the overall coordination of care and health services provided to an assigned caseload of residents ensuring compliance with licensing and funding regulations. This includes following each resident's individual service plan, directing and overseeing the care provided by Resident Care Assistants, contact with physicians and other care providers, follow through on implementation of physician orders, and documentation of care/services provided.

Qualifications:

Is able to advocate within the Resident First philosophy
Ability to demonstrate adherence to the Felician Core Values and to foster a corporate culture that promotes adherence to the Core Values by all staff
Current, unrestricted State of Wisconsin LPN license
1 to 2 years of geriatric experience; preferably in an assisted living setting
Excellent clinical knowledge and skills; skill in recognizing the physical, mental and emotional needs of residents.
Demonstrated ability to analyze issues, identify options and implement solutions.
Leadership skills
Outstanding oral and written communication skills; ability to maintain professional and cooperative relationships with all customers-family, other professionals, residents and staff from all departments.
Detail oriented, ability to manage multiple priorities in a high pressure environment
Familiarity with DHS 83 preferred
Basic computer proficiency necessary to utilize email and resident care software.
