Job Details

The Clinical Pharmacist oversees the medication distribution system for their area of responsibility. In addition, the Clinical Pharmacist identifies, prevents, and resolves drug therapy problems by ensuring each medication is indicated, effective, safe, and convenient for the patient. This position is for a full-time, day shift, unit-based/decentralized clinical pharmacist position on an internal medicine service.

Job Description

Key Responsibilities:

  • Serve as a clinical pharmacist on internal medicine inpatient services.

  • Participate in interdisciplinary rounds to provide real-time drug therapy recommendations.

  • Review and verify medication orders for appropriateness, safety, and cost-effectiveness.

  • Conduct medication reconciliation, discharge counseling, and care transitions support as needed.

  • Document clinical interventions, medication-related problems, and adverse drug reactions.

  • Educate and mentor pharmacy residents and pharmacy students.

  • Train staff as assigned (eg, residents, interns, technicians, new pharmacists).

  • Contribute to formulary management/drug information and support hospital policies and protocols.

  • Opportunities to lead/participate in departmental initiatives, research/quality improvement projects, and relevant committees.

Qualifications

* Doctor of Pharmacy (PharmD) or Bachelor of Science (BS) degree from an ACPE-accredited college of pharmacy.

* Registered Pharmacist license in the State of Pennsylvania

* Preferred: Completion of a PGY1 Pharmacy Residency

* Preferred: Board Certification (e.g., BCPS) or certification eligible within 2 years
