Job Summary Facilitates the referral of patients from a variety of settings- acute care, post-acute services, physicians, private payors and patients/families. Provides screening, assessment, planning, and coordination of services for the department. Maintains knowledge of federal and state rules and regulations governing home care to patients as well as individual private payor requirements. Excellent communication skills contribute to positive customer service and coordination of care. Works closely with the referral team to provide right time/right services patient care.
Essential Functions

Collects and processes data efficiently and accurately from a variety of sources including telehealth monitoring, to ensure clinician access to information is timely and to facilitate initiation of home health for patients referred to the agency. Coordinates planning services for requesting providers to include screening for high-risk factors, facilitation and coordination of referral of patients' care to other department services. Communicates patient information necessary for establishment of a plan of care to clinicians as appropriate, notifying supervisory personnel of impending referrals. Communicates and coordinates with referral team the scheduling need on every incoming referral and reviews it for medical necessity. Screens the patient for admission to home care based upon provider guidelines and/or criteria established by the agency, ensures patient is appropriate for Home Health services. Develops and maintains working relationships with referring physicians, social workers, case managers and others to promote and facilitate the referral and admission of patients. Maintains statistical information regarding admissions and patient referrals, generating report to the agency leadership. Ensure an attentive and quick response to our referral sources as well as WQ's to accept/denial patients. Timeliness expectation to have these cleared by end of business day for next day. Participates in performance improvement activities within agency and collects and records statistical data for the completion of reports regarding staff productivity, patient services, and other Home Health services operations. Performs other cross-trained role functions within agency as staffing needs requires and as assigned including, data entry, filing, faxing, copying, mail handling, scanning, and printing functions. Performs other duties as assigned.

Minimum Requirements
Education - High school diploma or equivalent Experience - One (1) year health care, business or nursing experience. Nursing experience in a home health setting preferred.

Job related licensure or certification such as LPN (Licensed Practical Nurse), RN (Registered Nurse), CNA (Certified Nursing Assistant), CMA (Certified Medical Assistant), PTA (Physical Therapy Assistant), or COTA (Certified Occupational Therapy Assistant).
