Job Summary and Responsibilities Acentra Health is hiring a Pre-Admission Screener to join our growing and dynamic team! Job Summary Join Acentra Health as a Pre-Admission Screener and make a meaningful impact through vital, purpose-driven work. In this fully remote position, you will play a critical role in evaluating individuals with potential Serious Mental Illness (SMI) who are being considered for admission to Medicaid-certified Skilled Nursing Facilities. Using your clinical knowledge and interpersonal skills, you will conduct phone screenings, gather essential documentation, and determine the appropriate status of Level II PASRR referrals-all while ensuring compassionate care and timely service delivery.
