Acentra Health is hiring a Pre-Admission Screener to join our growing and dynamic team!

Job Summary

Join Acentra Health as a Pre-Admission Screener and make a meaningful impact through vital, purpose-driven work. In this fully remote position, you will play a critical role in evaluating individuals with potential Serious Mental Illness (SMI) who are being considered for admission to Medicaid-certified Skilled Nursing Facilities. Using your clinical knowledge and interpersonal skills, you will conduct phone screenings, gather essential documentation, and determine the appropriate status of Level II PASRR referrals-all while ensuring compassionate care and timely service delivery.

Job Responsibilities

  • Proactively monitor the internal CSR call queue and provide timely assistance for inbound calls as needed.

  • Conduct outreach to healthcare facilities to verify individual statuses, request necessary documentation, and coordinate PASRR-related activities.

  • Maintain accurate, thorough documentation in both the state database and Acentra's internal tracking system, in alignment with contractual requirements and deadlines.

  • Manage, triage, and coordinate incoming calls and referrals related to the PASRR program with professionalism and urgency..

  • Develop and maintain a strong working knowledge of Acentra Health's internal policies, workflows, and department operations.

  • Utilize automated systems and databases to retrieve, review, and accurately log screening information.

  • Apply clinical judgment to assess individuals for potential serious mental illness using established criteria and risk factor knowledge.

  • Collaborate effectively with team members and cross-functional departments to support overall workflow and continuity of service.

  • Communicate clearly and professionally with individuals, facilities, and colleagues via phone, email, and written documentation.

  • Prioritize, multitask, and manage time efficiently in a high-volume, deadline-driven environment.

  • Adhere to all corporate policies and regulatory guidelines, including strict compliance with HIPAA, privacy, and security protocols.


Qualifications

Required Qualifications, Knowledge and Experience

  • Bachelor's Degree in a healthcare-related field preferred.

  • Proven ability to assess situations, gather information, and solve problems effectively-both independently and in a team setting.

  • Demonstrated ability to remain calm, empathetic, and solutions-focused in challenging or emotionally sensitive situations.

  • Strong customer service orientation-able to respond promptly to inquiries, seek feedback to improve service, and uphold service commitments.

  • Must be comfortable with the assigned work schedule, which includes weekend shifts, holiday coverage, and standard hours of 8:30 AM to 5:30 PM PST. Candidates must reside within the Pacific, Mountain, or Central time zones

  • Prior experience in a high-volume call center or remote healthcare environment is highly desirable.

  • Familiarity with call center technologies and customer relationship management (CRM) systems

  • Strong attention to detail with the ability to prioritize effectively and execute tasks with accuracy and efficiency in a fast-paced environment

  • Tech-savvy and comfortable using web-based platforms and software tools to conduct screenings and manage workflow.

  • Excellent verbal and written communication skills with the ability to engage effectively with diverse stakeholders.

  • Proficiency in Microsoft Office applications including Word, Excel, Outlook, and PowerPoint.

  • Experience working with contracts, providers, or managed care entities is a plus.

Compensation

The compensation for this role is $19.50 - $20.00.per hour

Based on our compensation program, an applicant's position placement in the pay range will depend on various considerations, such as years of applicable experience and skill level.
