Community Health Worker - Amputee Center of Excellence

Amputee Center of Excellence at SRH is committed to revolutionizing the care and support system for individuals with limb loss by focusing on Wellness, Education, Clinical Care, and Research. Through comprehensive education programs, we aim to equip our team with the skills and understanding necessary to offer the highest standard of care. Our goal is to empower amputees, foster innovation in treatment, contribute to the advancement of knowledge in amputee rehabilitation. Additionally, we strive to promote the well-being of our patients by addressing health-related social needs, system navigation, and care coordination as standard care. Community Health Workers (CHWs) are an integral part of achieving these goals. CHWs are trusted members of the community who help patients improve access and coordinate their health care. They possess the skills and experience to understand their patients' circumstances. By building trusting relationships and walking alongside their patients, CHWs help address medical and psychosocial needs to promote self-efficacy, help patients meet their goals, and improve health outcomes.
These objectives are achieved through proactive patient outreach, engagement, and care coordination. The CHW role collaborates closely with the interdisciplinary care team to connect patients to resources, improve patients' access to care, and support patients in achieving optimal health and well-being. The position requires a high degree of flexibility, independence, and willingness to participate in various activities while supporting all members of the ACE team.

Key Responsibilities:
  • Patient Advocacy and Support: Provide guidance and support in navigating healthcare systems, accessing community and government funded programs, and achieving individualized health and rehabilitation goals.
  • Care Coordination and Collaboration: Work closely with the interdisciplinary Amputee Center of Excellence (ACE) team to ensure comprehensive care, including proactive patient outreach, engagement, and connection to community and healthcare resources to promote optimal health and well-being.
  • Education and Empowerment: Participate in and contribute to the development of education programs designed to empower individuals with limb loss, enhance team knowledge, and advance innovative approaches in amputee rehabilitation and overall patient care.

  • Dedication to advancing the field through education, research, and advocacy.
  • Additional Job Details (if applicable)
  • High school diploma or GED required
  • Relevant prior work experience, OR associate or bachelor's degree preferred
  • Minimum two years of work experience
  • Previous work in community settings preferred

  • Ability to work independently and within a multi-cultural team
  • Demonstrated organizational and time management skills
  • Demonstrated ability to work effectively and provide advocacy for racially and ethnically diverse populations and communities preferred
  • Strong commitment to social justice and health equity
  • Experience working as a patient navigator/community health worker preferred
  • Strong oral and written communication skills
  • Proficient in all Microsoft Office Suite, including Outlook and Excel
