Overview

Presbyterian is seeking a Nurse Navigator to provide direct patient care to an assigned group of patients in an Outpatient Clinic. The Nurse Navigator is a patient advocate and is responsible for facilitating and following the cancer patient from point of diagnosis to treatment and then coordinating arrangements for survivorship care or providing quality of life care for cancer patients. The navigator is the contact person for the patient/family and serves as the liaison to all physicians and healthcare staff providing care and support for the patient

Qualifications

  • Associates of Nursing with experience in Oncology
  • Bachelor of Science in Nursing (BSN) preferred
  • RN-NM
  • BLS Required
  • ONS NN education within two weeks of hire.
  • Chemo-bio cert within 6 months of hire.
  • APHON-requirement for Peds Dept.


Responsibilities

  • Facilitates the entry of patients into the cancer program or assumes quality of life care of existing cancer patients with a late-stage diagnosis and coordinates care including family involvement. Follows the patient throughout the continuum of cancer care services across multiple departments.
  • Implements risk assessments, healthcare and quality of life pathways for cancer patients, based upon the clinical staging of the patient. Evaluates progress of patients along the continuum and is responsible for professional and personal modifications along with healthcare team as appropriate. Develops relationships with cancer physicians, including but not limited to: Radiologist, General Surgeon, Surgical Oncologist, Medical Oncologist, Radiation Oncologist, Pathologist, Plastic Surgeon, Primary Care Physician, Palliative Care, Hospice and serves as liaison between patient and physicians.
  • Prepares for and coordinates the weekly cancer multidisciplinary conference.
  • Provides access to community resources and links patients to support services such as: social work, nutrition, psychologist, pastoral care, physical therapy and rehabilitation, financial counseling, integrative medicine, pharmacy, legal services, palliative care, hospice, etc.
  • Assists research nurses and medical oncologists with accruing eligible cancer patients to clinical trials.
  • Assists with tracking of metrics and quality indicators for diagnosing and treating cancers promptly and effectively.
  • Works with cancer survivors to determine a survivorship program and develops a long-term plan for patients, along with other healthcare team members.
  • Perform other functions as assigned.
