Schedule Details:Monday-Friday 8am-4:30pm. This is NOT a remote position, employee is required to work on site at Waukesha Memorial Hospital.


What You Will Do:

As the RN Coordinator for Remote Patient Monitoring (RPM), you'll play a critical role in advancing patient care through innovative technology and care coordination. This is an onsite in person position where you will support your team and directly interact with patients to improve outcomes.

You will:



  • Coordinate the daily workflow of the RPM team, ensuring smooth operations and seamless patient care.
  • Educate and empower patients on proper use of RPM devices, such as blood pressure monitors, scales, and other tools.
  • Monitor patient data and escalate abnormal readings based on clinical guidelines.
  • Serve as a liaison within the organization, marketing and promoting the benefits of RPM services.
  • Provide patient care, including assessment, triage, and goal-setting, using motivational interviewing techniques.
  • Ensure compliance with clinical guidelines, documentation standards, and billing protocols.
  • Collaborate with leadership to enhance program infrastructure and drive growth.
