The ideal candidate will possess the following qualifications:

• A bachelor's degree in the psychology, social work, nursing, rehabilitation, counseling or other related human services field
• Must be fluent in English and Spanish
• Experience in the human services field working with children preferred
• Must have a valid driver's license and own a reliable means of transportation

We are seeking a Bilingual Blended Case Manager to join our team at our program in Philadelphia, PA.

Position Details:

The Bilingual Blended Case Manager (BCM) position is responsible for developing, coordinating, and implementing services to assigned consumers as identified in their service plan. The Bi-Lingual BCM will work and assist their assigned consumers by focusing on the following life domains: behavioral and physical healthcare needs, family functioning/independent living, educational and vocational goals, and socialization.
The Bilingual BCM must be fluent in Spanish (speaking/reading) to provide services to children and families whose primary language is Spanish.

Earn: $23.41 per hr.
