Your Role on the Team

Position Summary:

Incumbent serves as a patient educator-advocate. The Coach will empower patients to develop self-management skills with their chronic disease such as diabetes and heart failure through improved communication, education and empathetic coaching. The goal of the Community Coach is to help patient and their family or significant other to cope with their condition, manage symptoms and improve function and quality of life. The Coach will provide and/or support health care services to individuals with these chronic diseases at UC Irvine HealthCare primary care ambulatory and Cardiology sites as part of the health care team.

Required Qualifications:

  • Tact, discretion, and experience in handling confidential information
  • Proven commitment to and ability to provide high-level customer service in attitude and work ethic
  • Personal organization skills to keep track of all task, projects, and relevant action items
  • Must possess the skill, knowledge and ability essential to the successful performance of assigned duties
  • Must have valid California Driver's License or the ability to obtain one and/or own transportation commute to clinic sites
  • Must demonstrate customer service skills appropriate to the job
  • Knowledge of Diabetes or Heart Failure disease
  • Excellent written and verbal communication skills in English
  • Excellent organizational skills and attention to detail
  • Competent knowledge and experience with Microsoft Office computer applications, i.e. Word, Excel
  • Attention to detail, ability to follow complex verbal instructions, and commitment to quality of output
  • Ability to work in challenging environment with frequent interruptions, changing priorities, and multiple deadlines
  • Ability to maintain a work pace appropriate to the workload
  • Ability to establish and maintain effective working relationships across the Health System
  • Ability to be forward thinking and anticipate problems and solutions
  • Ability to be a team player, yet work independently to accomplish tasks

Preferred Qualifications:

  • Will train - UCIMC electronic medical record training, (EPIC), patient scheduling, HIPAA training
  • Will train - Disease management protocols including the protocol for working with patients as a community health worker
  • Experience working on human subject research
  • Experience working in clinic settings
  • Bilingual in English and Spanish
