Mgr of Care Navigation

Summary:

Under the general supervision of the Director of Case Management the Care Navigation Manager will have an opportunity to work with a multidisciplinary team across the Brown University Health system to assist with patient throughput. This role will oversee a team that is responsible for coordinating smooth transitions in and out of the BUH system. The Care Navigation Manager will work closely with the BUH Capacity Management and Access Center to review incoming transfers and to oversee repatriation efforts. The Care Navigation Manager will oversee post-acute case management programs including oversight and coordination of care for the BUH paramedicine program.

Responsibilities:

Provides leadership for the Nurse Care Navigator team as well as BUH pre/and post care coordination programs. Assists in the development implementation and evaluation of orientation and continuing education. Participates in the interviewing process. Recommends selection of staff directs activities and evaluates professional and support staff to optimize achievement of goals and objectives.

In collaboration with the Director of Case Management develops measurements for evaluating staff competencies and orientation. Acts as a resource to staff. Develops department policies and strategic plans.

Develops analyzes and adheres to departmental capital and operating budgets ensuring appropriate staffing and resources are in place to support department standards of practice. Provides for the effective utilization of resources (people financial material and equipment) to meet established goals and objectives.

Acts as a clinical consultant for all members of the healthcare team regarding complex issues in areas of case management (such as hospital location appropriateness of admission and continued stay denials third party payor discharge planning).

Effectively and continually communicates with the staff nursing leadership and outer department managers to facilitate the flow of information necessary for the success of employees department families and the organization.

Participates in or leads various committees task forces and quality improvement teams as needed.

Facilitates continuous performance improvement by encouraging innovation supporting reasonable risk-taking and maintaining openness to new ideas.

Works with staff to reviews pending transfers and identify discharge barriers and the potential for repatriation once the patient no longer requires tertiary care and is determined to be safely returned to their hospital of origin. Escalates barriers in repatriation or early discharge barriers to Case Management and Capacity Management and Access Center leadership.

Assists with repatriation by proactively working with staff to identify candidates across the system to prepare and initiate this process.

Attends team meetings and/or capacity huddles with Capacity Management and Access Center and Case Management teams as indicate.

Assists with education on Repatriation and with Case Management Staff orientation as needed.

Monitors system-wide capacity and develops strong knowledge of Brown university Health and community resources available for patients. Understands Brown University Health affiliate capabilities as well as the capabilities of other hospitals in Rhode Island and Massachusetts.

Documents all activities in accordance with department policy. Assists with data collection.

Promotes a system mindset to identify ways to support patients and improve throughput across the system Acts as a change agent by identifying opportunities to improve patient flow and reduce service delays through problem resolution and follow-up.

Adheres to Brown University Health Hospital and departmental policies and procedures to maintain a safe environment of care and compliance with all laws regulations and standards.

Participates in BUH Case Management On-Call rotation.

Skills

Interpersonal skills necessary to establish and maintain effective working relationships communicating with disciplines across an entire system.

Willingness to work with Capacity Management and Access Center leadership and Case Management leaders to improve existing processes and design new ones when needed.

Demonstrate good prioritization skills and the ability to manage multiple responsibilities.

Ability to work independently in a complex setting while also collaborating appropriately with a multidisciplinary team.

Engagement in hospital-wide capacity challenges and has a good understanding of the downstream impacts to patient flow across the system

Other information:

Supervision

Direct or indirect supervisory responsibility for up to 20 FTEs.

Supervision includes interviewing hiring scheduling training conducting performance appraisals recommending personnel actions and as necessary corrective actions.

Qualifications/Education

Must have current licensure to practice as a Registered Nurse in the State of Rhode Island

Bachelor*s degree in Nursing from an accredited National League of Nursing (NLN) program with current license to practice as a Registered Nurse in the State of Rhode Island required.

Masters degree in Nursing or related field preferred.

Certification by a recognized national organization in case management is preferred; or certification to be obtained within eighteen months of hire.

Demonstrated knowledge and skills necessary to provide care to patients throughout the life span with consideration of the aging process human developmental stages and cultural patterns in each step of the care process

Three to Five years of clinical case management experience in an acute care hospital required.

Three to Five years of Case Management or Capacity Management Leadership preferred.

Minimum 5-10 years of acute care nursing experience required.

WORK ENVIRONMENT AND PHYSICAL REQURIEMENTS

Works in a normal office environment with exposure to patient care.

Must be able to lift and transport ten pounds when moving equipment to various areas in the hospital.

General hospital environment with occasional stressful conditions associated with patient care.
Risk of exposure to blood borne pathogens and communicable disease is minimized and controlled by adherence to Hospital Infection Control policy and procedures.
Must be able to make hospital rounds through various patient care areas either by walking or through some other mobile means.
Visual acuity and finger dexterity is needed to review medical records navigate through automated system screens and type on a typical computer terminal keyboard.
Must be able to lift and or carry up to 10 lbs. Must have reliable transportation.
