Nurse Care Navigator

Summary:

Under the general supervision of the Manager of Care Navigation or Manager/Director of Case Management the Nurse Care Navigator will have an opportunity to work with a multidisciplinary team across the Brown University Health system to assist with capacity management and will play a lead role in repatriation. The Nurse Care Navigator will be highly effective in clinical fact gathering and analysis to facilitate appropriate pathways to optimize capacity. The Nurse Care Navigator will coordinate closely with the Capacity Management and Access center team Case Management Team physician administrator on call and with staff at non-Brown University Health organizations to ensure timely access to care and efficiency in patient flow. Nurse Care Navigator will collaborate with the clinical healthcare team across the patient care continuum and participates in the escalation of complex patients to case management leadership.

Responsibilities:

Supports all throughput and capacity management activities for the Brown University Health System.

Reviews transfer requests from other acute care hospitals as well as direct admit requests.

Reviews all out of state transfer requests and communicates any barriers related to insurance or discharge planning to Epicenter and case management leadership patient were to be accepted.

Reviews pending transfers to identify discharge barriers and the potential for repatriation once the patient no longer requires tertiary care and is determined to be safely returned to their hospital of origin. Escalates barriers in repatriation or early discharge barriers to Case Management and Capacity Management and Access center leadership.

Assists with triaging patients coming in from PCP offices and collaborates with the outpatient case management teams to facilitate care coordination from the community when appropriate.

Assists with repatriation: proactively identify candidates across the system and works with unit-based case manager as well as providers to prepare and initiate this process.

Attends team meetings and/or capacity huddles with Case Management and Capacity Management and Access center teams as indicated and helps provide education to hospital-based Case Managers on Repatriation. Assists with Case Management Staff orientation as needed.

Provides case management consultation within and with external hospitals. Collaborates with hospital and community-based Case Managers and Capacity Management and Access center leaders and staff including physicians and nurses.

Monitors system-wide capacity and develops strong knowledge of Brown university Health and community resources available for patients. Understands Brown University Health affiliate capabilities as well as the capabilities of other hospitals in Rhode Island and Massachusetts.

Documents all activities in accordance with department policy. Assists with data collection.

Promotes a system mindset to identify ways to support patients and improve throughput across the system Acts as a change agent by identifying opportunities to improve patient flow and reduce service delays through problem resolution and follow-up.

Adheres to Brown University Health Hospital and departmental policies and procedures to maintain a safe environment of care and compliance with all laws regulations and standards.

Skills

Interpersonal skills necessary to establish and maintain effective working relationships communicating with disciplines across an entire system.

Willingness to work with Epicenter leadership and other Case Management leaders to improve existing processes and design new ones when needed.

Demonstrate good prioritization skills and the ability to manage multiple responsibilities.

Ability to work independently in a complex setting while also collaborating appropriately with a multidisciplinary team.

Engagement in hospital-wide capacity challenges and has a good understanding of the downstream impacts to patient flow across the system

Other information:

Education

Bachelor*s degree in Nursing from an accredited National League of Nursing (NLN) program with current license to practice as a Registered Nurse in the State of Rhode Island required.

Certification by a recognized national organization in case management is preferred; or certification to be obtained within eighteen months of hire.

Experience

Three to five years of clinical case management experience in an acute care hospital required.

Experience with transfer and access and/or repatriation preferred.

Minimum 5 years of acute care nursing experience required preferably in critical care or the emergency department.

WORK ENVIRONMENT AND PHYSICAL REQURIEMENTS

Currently Monday-Friday with opportunity for evening and weekend coverage in the future based on organizational need.

Works in a normal office environment with exposure to patient care.

Must be able to lift and transport ten pounds when moving equipment to various areas in the hospital.

General hospital environment with occasional stressful conditions associated with patient care.
Risk of exposure to blood borne pathogens and communicable disease is minimized and controlled by adherence to Hospital Infection Control policy and procedures.
Must be able to make hospital rounds through various patient care areas either by walking or through some other mobile means.
Visual acuity and finger dexterity is needed to review medical records navigate through automated system screens and type on a typical computer terminal keyboard.
Must be able to lift and or carry up to 10 lbs. Must have reliable transportation.
