JOB DESCRIPTION:

SUMMARY OF POSITION

The Life Skills Instructor provides care, oversight and training in all aspects of independent living skills to adults presenting with significant functional and behavioral deficits related to severe mental illness.

Provides care, oversight and training in all aspects of independent living skills to adults presenting with significant functional and behavioral deficits related to severe mental illness. The Life Skills Instructor provides structure, monitoring and advocacy services to residents in a group home supervised residential setting. As part of Centerstone team-oriented philosophy, the Life Skills Instructor is expected to actively participate in variety of cross-functional teams and performance improvement initiatives

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Supervises residents during structured and unstructured time to ensure safety of the residents and staff and models appropriate behavior.
  • Assists client in building a natural support team. Integrates natural community supports/resources, for any and all life domain area(s) identified as needing improvement, into the service delivery process.
  • Provides medication training and monitoring to support client personal growth and to assess client compliance and efficacy of medication, as indicated on the client's personal growth plan.
  • Assists clients with achieving their personal growth goals. Throughout the personal growth process, ensures services provided are driven by client personal growth goals. Links all clinical documentation to identified personal growth goals and objectives as indicated on the Individual Personal Growth Plan (IPGP). Ensures documentation of Progress Contact Note (PCN) activities reflect personal growth plan objectives and demonstrate medical necessity. Utilizes PCN completion as a personal growth tool to celebrate client successes and identify growth areas by completing progress notes concurrently with clients. Clinical documentation indicates active client involvement services (i.e. documentation is in the words of the person served).
  • Ensures a complete clinical record, completes Progress Contact Notes for all contacts with clients and collaterals within established timeframes.
  • Presents relevant information for the purpose of assuring quality of care, gaining feedback, and planning changes in provision of personal growth-based services. Identifies and reports all critical incidents according to policy. Demonstrates an ability to manage crisis situations, including danger to self and others, to ensure the safety and stabilization of clients and to make appropriate referrals/linkages when clinically indicated. Does not leave crisis situation until client is in a safe environment or crisis is resolved.
  • Demonstrates the ability to help the client identify, select and practice strategies allowing the client to develop the knowledge, skills and/or attitudes required for successful relapse prevention and attainment of personal growth goals. Demonstrates ability to do this by taking into consideration such characteristics such as disability, gender, sexual orientation, developmental level, ethnicity, age, spirituality and health status. Modifies approaches to engage clients or provide services as appropriate to the developmental/age-specific issues, cognitive ability, functional status, and/or diagnosis.
  • Communicates and coordinates with other social service providers and program staff regarding client personal growth progress.
  • Assists in the training of new employees and in-service training pertaining to position's specialty.
  • Encourages and facilitates personal growth and development toward personal growth and wellness. Empowers clients to be self-sufficient.
