We are a dynamic community-based social services organization dedicated to helping individuals and families reach their full potential through innovative programming tailored to diverse communities. We are seeking compassionate and dedicated professionals to join our team as Care Coordinators in our new and developing Youth Dual Care Coordination program. This program is focused on supporting children and their families in Milwaukee County who are facing challenges with severe mental health and/or substance use disorders, as well as clients with co-occurring developmental and/or physical disability needs. Dual Care Coordination programming follows the wraparound model of service delivery to identify needed services and develop multi-disciplinary treatment plans that address both the strengths and needs of the family. As the Care Coordinator, you will manage a caseload of youth who are dually enrolled in Comprehensive Community Services (CCS) and Children's Long-Term Support (CLTS) programs. You will coordinate in-home and community-based services to support the families' individualized needs. KEY RESPONSIBILITIES OF THE CARE COORDINATOR:
LOCATION: West Allis office with flexibility for hybrid/remote work scheduling after training is complete. Local travel is required to meet with clients within the home and other community spaces, with mileage reimbursement. Must be available to meet in-person with clients in Milwaukee County, Wisconsin. SCHEDULE: Monday-Friday 8:00am-5:00pm, with occasional evenings to accommodate families' schedules. SALARY: Candidates with a Bachelor's Degree: $24.00-26.00 per hour Candidates with a Master's Degree: $27.00-30.00 per hour WHY WORK WITH PSG/CIP?
