Your Role on the Team

Position Summary:

The incumbent serves as a technical leader and expert in social work education, blending the roles of educator and community-facing specialist in advanced care planning (ACP). This position delivers comprehensive educational services, including psychosocial interventions, advocacy, and community engagement, while leading formal and informal teaching for healthcare professionals, patients, and the broader community. Operating with a high level of autonomy, the incumbent drives strategies, administers policies, and oversees education initiatives that shape End-of-Life program development. Additionally, this role provides BBS-aligned supervision for ASWs pursuing licensure and conducts needs analyses for health promotion and education on End-of-Life issues, including advanced care planning, to enhance value-based care and improve service delivery.

Required Qualifications:

  • Working knowledge of the DSM-5.
  • Proven analytical and problem-solving skills to evaluate complex issues and identify options for resolution.
  • Must possess the skills, knowledge and abilities essential to the successful performance of assigned duties
  • Minimum of 2 years of supervising Associate Social Workers.
  • Knowledge of and ability to work within a complex health care system
  • Experience in delivering health promotion and or education programs.
  • Excellent written and verbal communication and interpersonal skills to effectively collaborate with various stakeholders, maintain flexibility, and work productively in a team environment with a diverse population.
  • Demonstrates expertise in advanced care planning, including facilitating discussions and guiding patients and families through decision-making processes.
  • Current Clinical Social Work License issued by the Board of Behavioral Sciences.

Preferred Qualifications:

  • Bilingual in English and Spanish

Licenses and Certifications:

  • LCSW
